Fees and Policies

New Patients

Private pay rate for adults (18+) are $400 for a 45 min session, $250 for follow-up sessions (usually 20-30 min). I do accept Aetna and certain other insurances for now. See my Headway account for details!

I do accept Out of Network patients and will provide a “superbill” that you can submit to your insurance company for reimbursement. We do not submit claims directly.

Initial intake CPT codes: 99205 or 99204 + 90836

30 minute follow ups: 99214 + 90833*

45 minute follow ups: 99215 + 90836*

*All codes are pending Medical Decision Making (MDM) and complexity of the case

Returning Patients and Documentation

To ensure accurate and appropriate completion of FMLA, disability, or other work-related forms, a scheduled appointment is required. These documents involve clinical judgment and must reflect your current status. Paperwork will not be completed outside of a visit. Disability paperwork will only be completed for patients that are seen for 3 months or longer.

Cancellations and No-shows

If for any reason you are unable to keep your appointment, please notify me 2 business days prior to your appointment time. Except in cases of a medical emergency, you will be charged the entire session fee of your missed appointment.

All patients are given a 10 minute grace period to arrive for their appointment. Arriving after this time may result in a cancellation fee.
